If you wish to register with us please collect and complete the appropriate forms from reception, then simply bring along 2 forms of identity, such as your medical card, passport,driving licence or birth certificate and hand them all in to reception. You will then be given an appointment for a New Patient check with our HCA.
list of the areas covered by the practice is set out on the back of our practice information leaflet. If you have any concerns whether your or not your address is included in this area please ask a Receptionist who will be happy to help you.
You will be offered an appointment with the Practice HCA after you register. It is important that you keep this appointment, as it will help the Doctor to look after any problems you may have while we wait for your records to arrive from your previous Doctor.
USE THE INTERNET TO BOOK YOUR NEXT APPOINTMENT
Siam Surgery now accepts appointment booked online through EMIS Access. This new service allows you to view, book and cancel GP appointments via the internet 24 hours a day. Internet appointment booking is especially useful for those times when the practice is closed, or telephone lines are busy.
