Register as New Patient

If you live within our practice area (see below) you can register online using the link provided above, or by coming into the surgery and our reception staff will be happy to guide you through the procedure. You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with one of our nurses to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. You can either book this appointment when you hand in your forms or call reception after you have registered online, otherwise you will receive an invite within the first few weeks of your regsitration.

Medical treatment is available from the date of registration. All patients will be allocated a named GP at the point of registration, if you wish to choose which GP this is then every effort will be made to accomodate your preference.

Please note: We cannot accept any new registrations from patients living outside of our practice area. Any currently registered patients who move out of area will have to register at a practice local to their new address.

Change of Personal Details

If you change your name, address or telephone number, please update us as soon as possible through your NHS app/SystmOnline account or by emailing siamsurgery.admin@nhs.net.

You can also inform us filling out a paper form, available from our reception desk.  

Temporary Patient Registrations

If you are unwell while away from home you can receive treatment from the local GP practice by registering as a temporary patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. 

To register as a temporary patient please contact the reception team on 01787 322420 who will advise you further. You cannot register as a temporary patient here if you are also registered at another practice in the same area.

Non-English Speakers

Disabled Patient Facilities

Disabled patient facilities are available at Sudbury Community Health Centre. 

We have allocated parking spaces, ramps, a lift, wheelchairs and disabled toilets on site.
